The Permit-Required Confined Space (PRCS) Entry Form is required for entry into any permit-required confined space. The PRCS Entry Form may be used for either an alternative methods entry or a permit entry depending on the hazards present in the space and the ability to eliminate or control the hazards.
A person can be seriously injured, even killed, when working on a machine or equipment if the hazardous energy sources are not controlled.
When performing maintenance, service or repair work on equipment, processes or systems, all hazardous energy sources must be controlled using lockout/tagout procedures and OSHA-approved locks and tags to protect those impacted from the unintended start-up or release of stored energy. Common hazardous energy sources include electrical, hydraulic, pneumatic, mechanical, thermal, chemical or gravitational.

University units with personnel who work outdoors are required to implement safety measures to help prevent heat-related illness, including providing initial and annual training, and implementing a written Outdoor Heat Safety Plan.

The University’s COVID-19 Vaccination Policy is no longer in effect as of June 12, 2023.
Being up-to-date on COVID-19 vaccination is strongly encouraged for all UW personnel and students.
UW Medicine medical facility personnel are required to follow the UW Medicine COVID-19 vaccination policy.
University units can use the Supplemental Accident Prevention Plan (SAPP) Template (Word) to organize their worksite-specific health and safety information, and address employee protections from site-specific occupational hazards that extend beyond the University of Washington Accident Prevention Plan.
