UW Hazardous Waste Labels: Four different labels

Download an accessible form to create UW Hazardous Waste Labels.

This form that allows you to enter unique label information for four different hazardous waste containers.

1. Fill in the label information with the chemical compounds and corresponding percentages. Be sure to select the correct hazard type on each label.
2. Print the labels Avery 5168, 3M 3100, or equivalent. Be sure to select “Actual Size” in the print options menu.
3. Affix labels to the hazardous waste containers.
