I WANT TO REPORT ...
All work-related injuries, illnesses, exposures, fires, property damage and near-miss events involving UW personnel must be reported via the UW Online Accident Reporting System (OARS) within 24 hours of the incident; however, the following incidents require immediate notification to Environmental Health & Safety (EH&S):
- In-patient hospitalization, amputation, loss of an eye, or fatality
- Recombinant/synthetic DNA/RNA exposure or spill
- Radioactive material spill, exposure, accidental exposure from a radiation producing device or laser
During EH&S business hours (8:00 a.m. to 5:00 p.m., Monday to Friday) call 206-543-7262. Outside of EH&S business hours, call the UW Police Department at 206-685-8973 to reach EH&S on-call staff.
If you have issues submitting your OARS report, please contact EH&S at injury@uw.edu.
If you believe you have become sick from eating at a food service establishment at a University of Washington location, please submit a report (UW NetID required) or contact phdept@uw.edu.

Follow the guidance on the Spill Response and Exposure Response posters for all biohazardous, chemical, and radiological spills and releases.
Report sightings of pests and injured, sick, or dead animals at a University location.

Safety concerns may be reported to a supervisor, a safety coordinator, a health and safety committee representative, or to EH&S using the Report a Concern form.
Please provide details so the concern can be properly addressed. The Report a Concern form can be submitted anonymously.
A safety concern is a condition, practice, or situation that has the potential to result in physical harm, property loss, and/or environmental impacts. For example, moss on stairwell poses a slip hazard and should be removed before someone slips and falls.
The Report a Concern form must not be used in the case of a work-related injury, illness, exposure, fire, property damage, or near-miss event.

Concerns about air temperature, ventilation, natural gas and exhaust odors, construction dust and odors, and housekeeping issues that may affect indoor air quality can be reported to UW Facilities on the Seattle campus or your facilities manager at other UW locations.
