This reporting tool is for non-emergency incidents that occur on University of Washington owned or leased property that involve a University student, personnel, or visitor.

To report a workplace injury, illness, exposure, fire, property damage, or near-miss event, please submit a report through the Online Accident Reporting System (OARS).


Please give us as much information as you are willing or comfortable providing so that we can adequately address the issue.

Please note that Environmental Health & Safety (EH&S) will evaluate your report and share details with relevant parties as needed. Details of the incident may be shared, however we will make every effort to protect your personal information. Please remember that all information contained within this report may become part of the University's administrative record keeping pursuant to WAC 478-140-018.
Depending on the nature of the incident and the details provided, EH&S may or may not address the issue directly and we cannot guarantee a response to every reported incident.
