The information in the Food Worker Safety Focus Sheet educates food workers about foodborne diseases and how to reduce the risk of foodborne disease transmission.
Students may request a 90-day extension for a COVID-19 vaccine following a recent COVID-19 infection. Complete this COVID-19 Vaccination Extension Request form and return it to HSIP at myshots@uw.edu.

The University of Washington has multiple locations where KN95, KF94, N95 and surgical masks can be picked up through the 2023/2024 academic year.
Masks will be available to UW students and personnel at no charge on a first-come, first-served basis.
