Job Hazard Analysis Template

The Job Hazard Analysis form helps you identify the hazards of a specific task. After the hazards are listed, one can identify controls to mitigate risk.

A Job Hazard Analysis helps identify required trainings and personal protective equipment needed to stay safe while doing potentially hazardous tasks. It can also be used as a communication or training tool for new employees.

Follow the instructions for completing the Job Hazard Analysis.

First Aid Plan Guidelines (updated 9/17/21)

All University personnel must have access to quick and effective first aid in the event of an emergency. The First Aid Plan Guidelines (Word) contain instructions for creating a First Aid Plan for University of Washington (UW) personnel on UW campuses, UW-owned sites, UW-leased spaces, temporary field locations, and field trips that are under the control of University operations and staff.

Basic Electrical Safety

We rely on electricity, but sometimes underestimate its capability of causing injury. Even household current (120 volts) can stop your heart. UW personnel need to be aware of the hazards electricity poses, such as shock, fire and explosion, and either eliminate or control those hazards.
